Poor Navigation and Fatigue lead to Grounding
The Australian Transport Safety Bureau ("ATSB") has recently released its report into the grounding of Massive Tide on 29 August 2006. The Report concludes that the failure to follow good navigational practices resulted in the offshore tug/supply ship sailing directly into Rosemary Island. The ATSB considered that the failure to follow company procedures as well as poor fatigue management contributed to the incident.
